The application for the 2011 Escape From Alcatraz Triathlon is now available online.
1. Please the print the Application; you will need Acrobat Reader to download pages (free program)
2. CAREFULLY read entire form!
3. Complete ALL information requested on form (this includes your PMS#).
If you currently do not have a Pacifc Master Swimming permit (PMS#), please apply at Pacific Masters website.
4. Read and SIGN RELEASE WAIVER.
5. Mail your completed form and your signed waiver with your check (made out to The Dolphin Club) to:
